Support for disabled children and children with additional needs

Cynllun Cyfeirio


The Cynllun Cyfeirio referral programme supports children aged 2-4 with a disability and / or additional needs to take part in educational play and learning opportunities during term time in a pre-school registered setting.

Referrals are made by professionals, parents or staff in pre-school settings to a multi-agency panel for approval.

The scheme is funded by the Welsh Government Children and Communities Grant and Ceredigion County Council funds, and is managed by Mudiad Meithrin.





YMUNO


The YMUNO Scheme aims to support children aged 3-14 with additional needs (up to 18 if disabled) who need additional support to attend registered childcare settings, including out -of-school clubs, holiday play schemes and open-access play provision, to provide inclusive play and childcare opportunities in line with the Equality Act 2010.

The scheme is funded by Ceredigion County Council and managed by DASH Ceredigion.

Parents can ask for details at their local after-school or holiday club or open-access play session.


Tax-Free Childcare


With Tax-Free Childcare you can get up to £500 every 3 months (up to £2,000 a year) for each of your children to help with the costs of childcare. This goes up to £1,000 every 3 months if a child is disabled (up to £4,000 a year).

Employability Support Team


We support residents of Ceredigion who are not in employment, education or training with 1:1 employability mentoring support, upskilling, training, in-work support and much more. We work with a wide range of individuals including those with a work-limiting health condition, carers, women (and lone parents), diverse ethnic communities and many more.

ELITE Supported Employment


ELITE Supported Employment is a charity empowering disabled people or those at a disadvantage across South, Mid and West Wales. ELITE supports hundreds of disabled people, or people who face a disadvantage, every year with vocational opportunities, training and employment through to independence.Contact details: 01443 226664 or email: information@elitesea.co.uk


DWP Access to Work


Every Jobcentre is supported by a specially trained Disability Employment Adviser. They are available for Work Coaches to ask for advice on providing support for specific cases. They offer support and advice to disabled people and their families. In particular, they help disabled people get access to job-related resources and services.

DWP Access to Work: you can apply for:

· a grant to help pay for practical support with your work

· support with managing your mental health at work

· money to pay for communication support at job interviews




Scope


Scope offers a support to work programme for disabled people in England and Wales, who are looking for paid work. Disability equipment and modifications


Care and Repair

Care & Repair Cymru is a charity, working to ensure that older people can live independently in safe, warm and accessible homes. Disability Grant


There is a Disability Grant for disabled people to pay for disability equipment and modifications to enable them to maintain their independence in their home.



Technology Enabled Care

Technology Enabled Care uses technology to provide health and care services to people in their own homes, helping people to live independently and safely. An example is having a ‘lifeline’ which is a call button worn as a pendant.




Poor Vision / Hearing Loss


In Ceredigion there is a wide range of services available to help people with a sensory impairment maintain as much independence and control over their lives as possible.

Sensory impairment’ is the term used to describe disabilities such as:

Blindness/Visual impairment

Deafness/Hearing loss

Deafblindness (Dual sensory loss)

The Orange Wallet Scheme is a collaborative project funded by the Welsh Government as part of the All-Wales Autism Spectrum Disorder strategy.

AskSARA is an award-winning online self-help guide providing expert advice and information on products and equipment for older and disabled people.


AskSARA is easy to use. Simply:

AskSARA will produce a free personalised report providing:

Clear, tailored advice written by experts on ways to help with daily activities and staying independent in your home

An impartial list of products and equipment, specific to your needs, with information on where to get them

An option to save your report and share it with family, friends and care workers


Blue Badge

The Blue Badge Scheme is a European arrangement of parking concessions for people who are disabled or who have severe walking difficulties or severe cognitive impairment.


A blue badge is available to both drivers and passengers. The European scheme is intended to allow badge holders (the person to whom the badge was issued), to park closer to their destination.




Support for people with arthritis


Living with arthritis can bring about extra costs, and we know that for many people the cost-of-living crisis is only making things harder.

We can offer help and advice for those with arthritis and musculoskeletal (MSK) conditions.
